We are in network with most insurance companies including Providence, Regence Blue Cross, Anthem, MODA, United, Cigna, Pacific Source, First Choice, UMR, Multiplan and more. Please call your insurance to ascertain your individual plan benefits as benefits can vary greatly from plan to plan. Legacy Health Employees, (pacific source), must see only Legacy Providers. Kaiser Permanente members must see only Kaiser providers unless you have the top tier plan that allows you to see providers outside of Kaiser.
Unfortunately we are not able to take Oregon Health Plan or Medicare at this time.
PLEASE NOTE: THERE IS A $150 NO SHOW FEE, AND A $200 NO SHOW FEE FOR WEEKEND APPOINTMENTS.
If you are a new patient, please note that if you schedule online, new patient forms and a confirmation request will be sent via email. If you do not confirm your appt within 1 week prior to the appointment, we cannot hold the appointment for you. You must fill out the forms prior to your appointment in order to ensure that your appointment time will be reserved for you. We schedule 90 minute new appointment times. There is a $150.00 dollar charge for a missed new patient appointment on the weekdays, and a $200.00 dollar missed appointment for a weekend appointment.
